First Visit

Maintenance -> Problem -> Crisis. When you fail to do maintenance regularly, you will develop problems. If you ignore the problems, you will be hit by a crisis. If you value your teeth you will brush + floss daily. What happen if you stop maintaining your teeth? Dental and gum problems over time lead to major oral crisis. If you value your car, you do oil change and tires realignment regularly so the car will last a lot longer. Failing to do maintenance care will COST you more in the long run, financially and mentally. Try skipping oil change for a year or not brushing your teeth for a year!

People who value their health and wellness maintain their spine and nervous system by regular preventative chiropractic care, because they realize that the spine + nervous system control everything in human body: heart, lungs, thyroid, sinus, bladder, colon, stomach, sleeping, energy level, brain chemicals, immune system ...everything.

If you are in crisis or have been in crisis for months and years, then we will first work together to help you out of the CRISIS. When you are no longer in crisis and you are able to function better, we will then correct the underlying PROBLEM by corrective and stabilization care. Once your corrective care is completed, it is now your decision to either maintain the correction by periodic preventive chiropractic care (maintenance), or quit completely and risk having to start all over again in a few months or years.

Preventive / maintenance care is not something you HAVE TO DO. Just like brushing your teeth or taking daily shower or oil-change. You don't really HAVE TO DO it. You can wait for weeks or months until your breath stinks, teeth rotten, or molds grow on your armpits before showering or brushing teeth. People who get regular periodic maintenance / wellness care at Pangemanan Chiropractic do so not because they have to but because they ENJOY THE BENEFITS - better sleep, better energy, headaches-free, stiffness-free, painkillers-free, drug-free, not being crabby, and they can enjoy the things they love to do without pain (gardening, golfing, skiing, working, exercising, sleep, sex, traveling, etc.).

The purpose of YOUR FIRST VISIT is to find out three things:
(1) if you have subluxations or interference with your spine and nervous system
(2) if you can be helped by chiropractic care
(3) if we want to accept you as a patient

Health History & Examination

Save 10-20 minutes before coming to the office by downloading the new patient application forms online and have them filled out BEFORE coming to our office.

  • Click here to download the New Patient Application forms in Adobe Acrobat (PDF) format, or click here in Microsoft Word (DOC) format.
  • If you were injured in an auto-accident, please click here for a different Auto Injury Application forms. (Please be patient as the speed of download depends on your internet connection speed. Click "Open" and print the document for you to fill out). If the forms did not print out properly because of your printer's settings or because of outdated operating system or softwares, call our office to fax you a copy.
  • Available to read and download: health privacy consent (HIPAA) with Consent for Chiropractic Care, and Pediatric History forms.
  • Remember to bring your picture ID and other pertinent information (insurance card, previous MRI report, x-rays, etc.) into your initial appointment.
  • Our staff will then conduct a computerized full-spine sEMG Insight Subluxation Station digital spinal scan, and if necessary or indicated, followed with a Foot Levelers' digital feet scan to rule out potential structural problem with the foundation of the body - your feet.
  • Next, the doctor will perform several examinations from range of motion to orthopedic tests, from neurological tests to posture examination.
  • In most cases, you will NOT receive chiropractic treatment on the first visit. Exceptions to this rule include pregnant moms and babies. On your first visit, following your exam, scan, and x-rays, depending on how severe your case is, you may receive temporary pain relief with our muscle-stim and cryotherapy in our rehab area to reduce spasms, swelling, and other common symptoms of acute injury. The doctors at Pangemanan Chiropractic will only provide chiropractic care AFTER conducting a thorough review and analysis of your x-rays, exam findings, digital scan results, etc. Your specific condition may require us to take x-rays to either rule out more serious conditions or assist us in developing the most effective treatment plan for you. "If you don't see it, you're guessing it." The doctors never rush to analyze x-rays, neither will they make guesswork. They always take their precious time to analyze your x-rays or have them read by a qualified, board-certified, radiologist, if necessary for difficult cases. This usually take a day, in some rare cases, two or three days until a radiologic report is received.

REPORT OF FINDINGS (2nd Visit)

  • This is the special time we set aside to give you (and your significant other) a thorough explanation of the exam findings, what's wrong, whether or not we can help you, how long will it take, how much the corrective care plan will cost, and so forth. There are no additional charges for this report. The Report of Findings will take approximately 10-15 minutes, and it is highly recommended that you bring your spouse or significant other or a close friend at this point, because in critical times like this, it is important to have someone that you can count on to be there for you and be objective for you. Also, if your spouse is the one who makes financial decision in the family, being absent from the doctor's report simply mean that he or she will not be able to understand completely how serious your condition is, how bad your x-rays or scan look like, the number of treatments to correct your condition, etc.
  • If the doctor(s) accept you as a patient in this office, then you may receive treatment immediately following the report or the next business day.
  • Home Instructions ~ Prior to leaving, depending on the case, patients may be given home-care instructions on certain activities or procedures to be conducted at home / work. This may include ice or heat application instructions, avoidance of certain activities or positions, as well as home exercises and/or stretches. If you do the things we tell you to do, and avoid the things we tell you to avoid, there is no reason why you won't improve with our care.
  • Treatment options for your condition as well as various payment options will also be discussed during this time. About half of our patients have no insurance or do not use their insurance or have very high deductible insurance plans--which is the best insurance plan for today. Our Corrective Care Plan (financial plan) breaks down the cost of corrective and wellness chiropractic care to a very very affordable, easy plan with a valid credit card for guarantee. Just ask Melinda or Lisa.
  • Financial Portion:
    • We have a ZERO BALANCE POLICY. Payments are expected at the time of service.
    • In most cases, we'll be happy to check and verify your insurance coverage as a courtesy to you.
    • We accept most insurances, including Kentucky's state insurance, Humana.
    • You do not need a referral from another doctor or health professional to be seen by a doctor of chiropractic.
    • In most cases, your health insurance will cover the exam fee, x-rays (if necessary), treatments as well as adjunctive therapies. If you have chosen an insurance policy that requires you to pay out of pocket first until you meet a certain deductible (ie. $500, $2000, etc.), then most likely your insurance will start to pick up after you meet this agreed amount of that year.
    • We can arrange an affordable financial plan that break down your care into smaller monthly or quarterly payments. A valid credit card is required for guarantee. This option is available on a case by case basis.
